Children may not bring medication of any kind to school for self-administration. All medication including asthma inhalers must be delivered to our school nurse in unopened original packaging. The nurse is required to administer all medications, including over the counter items, only after a Department of Education 504 medical authorization form has been completed by parent/guardian and students physician.

Please notify the school nurse of all allergies and medical conditions, as well as, informing the classroom teacher in writing.

• 504 medical forms are available from the nurse or the main office. If student’s medical condition changes during the course of the school year, please provide the school nurse with updates.
